Datavant can deliver records to your patient portal, securely email them to you, or mail physical copies if you request paper records. Records and Imaging Requests
Digital Imaging
To request copies of digital imaging (x-ray, CT, MRI, etc.), use the online request form below. On the form, select “Other,” and specify the exact imaging needed (e.g., MRI, CT, X-ray).
McNamara Hand Records
OPA is the exclusive custodian of McNamara Hand, LLC records for patients treated between May 2024 and April 2026. If you were a patient of Dr. McNamara during this period, please use the online request form below to submit your request.
Third-Party Records and Imaging
OPA can only provide records and images created by OPA. If another provider or facility created the record or image, OPA cannot release it—even if a copy is stored in your OPA medical record. You must request it directly from the provider or facility that created it.
Please note: Records and imaging requests may take up to 14 days to process.
Confidentiality: OPA keeps your health information strictly confidential and will not release it without your consent.
Legal exception: Workers’ Compensation and your insurance carrier may access your health information without your consent when permitted by law.
Disability and FMLA Form Requests

- A credit or debit card for the $30 processing fee (per form)
- A valid photo ID for verification
