Medical Records Requests

We partner with a third-party firm, Datavant, to support faster turnaround times for records requests. Datavant can deliver your records to your patient portal, securely email them to your preferred address, or mail physical copies if you request paper records. Please use the online request form below to initiate your request.

Digital Imaging Requests
To request copies of digital imaging (X-ray, CT, MRI, etc.), click “Start Medical Records Request” below. On the form, select “Other,” and specify the exact imaging needed (e.g., MRI, CT, X-ray).

Third Party Records and Imaging Related Requests
If OPA has received records and images that patients have brought in from outside sources, the law prohibits us from releasing these to a third party.

At OPA your health information is kept strictly confidential and is not released without your consent. Please note that Workers’ Compensation and your insurance carrier may legally access your health information without your consent. 

Disability & FMLA Form Requests

Orthopedic Physicians Alaska has recently partnered with Datavant to complete disability and FMLA paperwork for our patients. This online request is for patients and authorized caregivers who need forms filled out for an employer, insurance provider, or benefits administrator.

Before You Begin

Please make sure you have:

• The correct form from your employer or insurance company

• A credit or debit card for the $30 processing fee (per form)

• A valid photo ID for verification

• A current browser (Chrome, Firefox, or Safari)
